Currently, many people experience problems with pinched nerves and complaints of spinal pain in Surabaya. However, not many people know about pinched nerve therapy and chiropractic as a solution that can be considered. Pinched nerve therapy and chiropractic are treatment methods that involve physical treatment of the bones. This method aims to relieve pain, restore normal spinal function, and improve body posture. In pinched nerve therapy, physical treatment is carried out to reduce pressure on the pinched nerve. This can be done through various techniques, such as spinal manipulation, massage, and muscle stretching. Meanwhile, chiropractic also includes in-depth research and diagnosis of conditions of the spine and nervous system.

Along with the increasing popularity of pinched nerve therapy and chiropractic, many clinics and practitioners offer these services in Surabaya. This provides an opportunity for people to receive holistic treatment and not just rely on drugs. With a deeper understanding of the benefits and procedures of this therapy, it is hoped that the people of Surabaya can use it as an option to overcome the problem of pinched nerves and spinal pain. However, this type of therapy also has certain risks and contraindications that need to be considered before deciding to undergo pinched nerve therapy and chiropractic. The following are contraindications for pinched nerve therapy and chiropractic that need to be considered:

  1. Bone or Disc Infections: Patients with spinal infections (osteomyelitis), disc infections (discitis), or general infections that can spread to the spinal area should not receive spinal manipulation due to the possibility of worsening the infection or its spread.
  2. Severe Structural Damage: This includes conditions such as spinal fractures or severe ligament damage, where additional manipulation of the area may worsen the condition.
  3. Progressive Neurological Conditions: Diseases such as myelopathy (damage to the spinal cord) or cauda equina syndrome (compression of nerves in the lower part of the spinal cord) must be treated with extreme caution and are usually not candidates for chiropractic.
  4. Severe Osteoporosis: People with high severity osteoporosis have more brittle bones that can break easily, including during manipulation.
  5. Significant Structural Spinal Abnormalities: For example, very severe scoliosis or unstable skeletal conditions such as spondylolisthesis (a condition in which one vertebra shifts away from the vertebra below it) may increase the risk of injury during manipulation.
  6. Vascular Disease: Abdominal aortic aneurysm or other vascular conditions that pose a risk of bleeding or stroke if certain manipulations are considered.
  7. Anticoagulant Use: Patients who use blood thinning medications or have blood clotting disorders may have a higher risk of internal bleeding after treatment.
  8. Cancer that Metastasizes to the Bones: Cancer that has spread to the bones can weaken the structure of the spine and increase the risk of fractures during chiropractic treatment.
  9. Rheumatological Conditions: Diseases such as poorly controlled rheumatoid arthritis or connective tissue conditions such as lupus that may affect the bones or joints may also be contraindicated due to the possibility of additional damage or increased pain.
  10. Abnormalities in the Blood Supply to the Brain: Such as vertebrobasilar insufficiency syndrome, in which the blood supply to the brain is insufficient, is especially concerning because neck manipulation techniques carry a risk, although very small, of causing a stroke.
  11. Unexplained Neurological Symptoms: Weakness, numbness, or other changes in sensation not explained by a clear diagnosis should be carefully evaluated before consideration for chiropractic manipulation.
  12. Allergic Reaction or Sensitivity: Although less common, sensitivity or allergy to techniques or ingredients used in chiropractic care, such as creams or oils used in therapeutic massage, should also be considered.
  13. In Pregnancy: There has not been much research on chiropractic in pregnant women. However, some people believe that manipulation of the spine during pregnancy may increase the risk of complications.

Chiropractic is a recognized form of health care and is integrated into the health care systems of many Western countries. The chiropractic profession has evolved and is seen as an alternative or complementary approach in the treatment of musculoskeletal pain, such as back and neck pain. Chiropractors in many areas of the West are primary care practitioners who require university level education and a license to practice.

In many Western medical systems, chiropractic is considered a nonsurgical and nonpharmacological treatment option for certain conditions. Spinal manipulation, which is a key technique in chiropractic, has been researched and supported by scientific evidence in some cases for the management of acute and chronic pain, such as low back pain.

The chiropractic profession advocates a holistic approach and works within a biopsychosocial framework, which emphasizes the need to understand the biological, psychological, and social aspects of a patient's illness or complaint. However, as stated in "Best Practices for Chiropractic Management of Patients with Chronic Musculoskeletal Pain: A Clinical Practice Guideline", there is an understanding that chiropractors must also be familiar with nonmanipulative therapeutic approaches and can refer patients to other health services if necessary, to manage conditions such as chronic pain (Hawk et al., 2020).

However, there remains discussion and skepticism in some aspects of the medical community regarding certain claims in chiropractic that may not be supported by strong scientific evidence. Therefore, collaboration between chiropractors and other medical practitioners, as well as ongoing research, is essential to gain a complete understanding and encourage greater acceptance in the Western medical world.

Chiropractic in Indonesia is not specifically regulated by the same health laws as other medical professions, and chiropractic practice is not yet fully recognized as in some Western countries. The absence of clear regulations often leads to gray areas regarding legitimacy and standards of practice. However, there are chiropractic practitioners who practice in Indonesia, usually in certain clinics or health centers that offer a variety of alternative or complementary health services.

It is important for people to ensure that the chiropractic practitioners they visit are sufficiently qualified, usually through education and training in countries where chiropractic is a legally recognized and regulated profession. Patients should also exercise caution and seek information about the credibility and track record of the practitioner before undergoing treatment, especially considering the low level of regulation of this practice in Indonesia.

Consultation with a doctor is also recommended before trying chiropractic to ensure that there are no medical contraindications and that it is the right choice, considering the patient's health and medical condition.
